Referring to the symptoms of sciatica pain and its treatment methods, a doctor of physical therapy said: According to the place where the sciatic nerve is involved, the types of treatments are different.

If the involvement of the sciatic nerve is caused by lumbar disc herniation, physiotherapy treatment is based on the reduction of disc inflammation and subsequent reduction of sciatica inflammation and pain in the lumbar spine.

Dr. Iman Kamali Hakim, Ph.D. in Physiotherapy, said: Involvement and stimulation of the roots of the sciatic nerve in the lumbar vertebrae or inside the pelvis and under the piriformis muscle causes symptoms such as pain, numbness, tingling, and tingling along the nerve path from the hips to the fingers.

The large and thick sciatic nerve branches from the joining of the lumbar plexus nerves in the lumbar spine and comes out of the sciatic hole in the pelvis and passes under the piriformis muscle in 80% of people.

He continued: In 20% of people, the nerve in the pelvis branches or splits, and one branch of the sciatic nerve passes through the piriformis muscle.
Then it extends from the back of the thigh to the fingers, or the whole nerve passes through the piriformis muscle, or it splits into two branches, one branch passes from the top and one branch from the bottom of the muscle.

In response to our reporter’s question about the cause of sciatica pain, Kamali Hakim stated: The most important cause of sciatic nerve pain and involvement is lumbar disc herniation, spinal canal stenosis in the lower back, bone spurs caused by joint arthritis in the lower back, damage to the sciatic nerve trunk due to Pressure, impact, crushing and piriformis involvement syndrome.

Referring to the signs and symptoms of sciatica, this physiotherapist said: The symptoms of sciatica include pain that starts from the back or hips, and sometimes extends to the toes.

In case of severe nerve involvement, numbness, tingling and tingling may also occur.

He pointed out how to diagnose sciatica pain and continued: specialized physical therapy and orthopedic examinations are needed to determine the location of involvement and entrapment of the sciatic nerve.

Most physiotherapists reach the final diagnosis after taking the history and necessary examinations.

But in some cases, it is necessary to perform tests such as MRI or radiology for the differential diagnosis of nerve involvement.

This specialist doctor of physical therapy said about the treatment of sciatica and its methods: In most cases, sciatica pain disappears by itself over time, but if it lasts for a long time, it is necessary to diagnose the exact location of the sciatic nerve involvement by the physical therapist in order to carry out the correct treatment. .

Dr. Kamali Hakim continued: According to the location of nerve involvement, the types of treatments are different. If the involvement of the sciatic nerve is caused by lumbar disc herniation, physiotherapy treatment is based on the reduction of disc inflammation and subsequent reduction of sciatica inflammation and pain in the lumbar spine.

He added: If the primary cause is spinal canal stenosis, physical therapy and powerful laser therapy, corrective exercises and lifestyle modification training are very useful and helpful.
This physiotherapist emphasized: In many cases, the cause of the involvement of the sciatic nerve is not in the back, but this muscle may become inflamed and painful in the pelvis due to spasm and cramping of the piriformis muscle, it is called piriformis pain syndrome, which of course is rare. Is.

Although there was a lot of emphasis on this syndrome before, today, physiotherapists have realized that most of the problems with the involvement of the sciatic nerve are caused by the involvement of the lumbar spine.

Dr. Kamali Hakim added: For the treatment of piriformis pain syndrome, specialized physiotherapy treatments including stretching exercises for the piriformis muscle, use of dry needling to open the piriformis muscle spasm, release technique of muscle-connective tissue, kinesiotyping and high-power laser therapy are recommended.
